How Home and School Can Work Together to Transition Students with Special Needs to New School

When it’s time to transition a student with special needs to a new school, it can be hard for students and their parents. Planning for this transition gives parents, educators and students a roadmap by which the student can prepare for the new school building. This flyer offers insight from a parent and a teacher on how each can support the student in a transition. Share this resource with educators and families!

The flyer was developed by Heartland AEA’s Parent & Educator Connection (PEC), a team of parents of children or young adults with special needs who encourage and promote positive partnerships between parents and educators so that children with special needs are successful.
